4 (10) hour shifts 10:30am to 9:00pm and one Saturday a month 9:00am to 7:30pm

5 (8) hour shifts 12:30pm to 9:00pm and one Saturday a month 9:00am to 5:30pm

 

Provides clinical assessment and health education to enrolled participants in the disease management program. Essential duties to include, but are not limited to: Applies knowledge of chronic disease, key interventions, clinical guidelines and clinical assessment to identify major health risk factors to provide education for proper health care. Maintains communication with participants enrolled in the disease management program in order to establish relationship and provide assessment, monitoring and health education. Conducts scheduled follow-up calls; develops goals and action steps to improve outcomes according to the key interventions and clinical guidelines. Completes documentation. Communicates with physicians and providers as needed. Consultant conducts participant disease management interventions per policy and/or protocol while meeting performance expectations established by management. Performs other duties as assigned.



Current unrestricted RN license from the Commonwealth of Virginia Board of Nursing required. 3 years recent pertinent inpatient or outpatient clinical experience or patient management experience required. Previous experience in adult or pediatric, med/surg, home health, or case management experience preferred. Previous experience with chronic conditions of diabetes, asthma, COPD or cardiac disease experiences and patient education experience preferred. Excellent oral and written communication skills as well as strong organizational and motivational skills required. Ability to communicate effectively with a variety of clients required. Good computer skills required.
