This field-based position offers a flexible, weekday schedule, and home-office resources will be provided - including a lap top, printer, cell phone and internet reimbursement. Primary responsibilities include:

 
Providing care to nursing home residents within established guidelines as stated by Evercare
Collaborating with physicians, facilities and patient families to ensure well-being of patients
Supporting Program Enhancement - providing referrals for other facilities as needed and working to meet educational needs within facilities
Communicating with sales team regarding leads and other pertinent information



Must be an NP currently licensed to practice in New York. Adult, family or geriatric certification is preferred. Nursing home, geriatric or family practice experience is also preferred. General computer proficiency in standard MS Office applications is required.
