Nurse/ RN Field Case Manager - Burlington, VT

This position will be responsible for providing case management services to assist individuals in reaching pre-injury level of function and/or maximum medical improvement. Activities include coordination of medical care and return to work. CCM and experience in either Case Management or home care preferred. Local travel is required (Central and Northern Vermont). Must have strong computer skills. Meets with clients in their homes; at physician's, therapist's, or attorney's offices; and/or at work sites to provide on-going case management services. Communicates both in person and by telephone with clients, medical providers, attorneys, employers, insurance carriers, etc. Prepares reports and other required paperwork as documentation of all casework activities at a consistently high standard and in a timely manner.
Understands and applies all special instructions required by individual insurance carriers and referral sources.
Understands and applies all laws and regulations which apply to the provision of rehabilitation services.

CURRENT RN LICENSE IN VT
3 to 5 yrs. of Nursing
1 to 2 yrs. of Case Management
1 to 2 yrs. of Workers Compensation preferred
