Nurse Team Leader (Disease Management)
SUMMARY
The Nurse Team Leader (Disease Management) plays a central role in the delivery of the FutureHealth program, including improving the healthcare outcomes of individuals in order to reduce the overall costs for clients. The Team Leader is responsible for the day-to-day supervision of the Disease Management team and works under the general supervision of the Director of Disease and Maternity Management.
RESPONSIBILITIES
1. Driving and managing the internal workflow of the DM Team.
2. Acquiring an in-depth understanding of the Disease Management product in order to analyze and make recommendations for improvement and growth.
3. Monitoring the team members' telephone technique to give ongoing recommendations for improvement to increase participation
4. Overseeing the day-to-day effectiveness and efficiency of the Disease Management functions, including monitoring of productivity and quality
5. Supervising the Disease Management nurses, including:
Assigning cases
Monitoring output
Reviewing cases
Training, coaching and counseling as appropriate
6. Participating in the development and maintenance of criteria and case management practice standards for use in selecting, interviewing and managing cases
7. Acting as an operational resource to determine how changes in other systems impact RiskScreen and disease management.
8. Troubleshooting individual cases, when appropriate, and serving as a clinical resource to nurses in developing approaches to handling calls and managing cases
9. Monitoring compliance with the Quality Assurance Plan developed for Disease Management
10. Providing input into the development and maintenance of program evaluation methodology.
11. Training of new Disease Management nurses, including determination of the need for training in other areas.
12. Participating in the professional development of the Disease Management nurses.
13. Managing his/her own caseload and Disease Management performing outreach.
14. Meeting or exceeding Disease Management participation and QA goals.
QUALIFICATIONS
1. A Registered Nurse licensed in the State of Maryland
2. Minimum 5 years experience in direct patient care providing patient education or case management
3. BSN
4. Minimum one year prior supervisory experience.
KNOWLEDGE, SKILLS AND ABILITIES
1. Ability to persuade potential members to participate in the disease management programs
2. Ability to motivate the nursing team to meet their challenging goals.
3. Ability to multi-task.
4. Assessment, evaluation and independent decision-making skills.
5. Analytical skills including deductive reasoning, college-level mathematics and statistics and intuitive cognition.
6. Ability to respond quickly to situations.
7. Demonstrated ability to show initiative in change/improvement efforts.
8. Ability to provide oversight, identify problems and create solutions for the Disease Management process.
9. Good organization and project management skills.
10. Ability to set performance objectives and monitor progress.
11. Ability to coach nurses on business, clinical and performance matters.
12. Excellent interpersonal and communication styles (clear diction, expresses self succinctly, effective listener, possesses good business writing skills, able to adjust vocabulary/technique to the audience).
13. Assertiveness and capability to address problems with providers, clients and team members, control the direction of care, and manage the time spent with each client.
14. PC skills, effective use of electronic systems and efficient keyboarding skills
15. Ability to apply the philosophy and values required to effectively counsel clients, despite any personal conflicts.
16. Ability to maintain the confidentiality of client and FutureHealth information.
