We are in search of a caring Registered Nurse with a passion for patient advocacy to join our supportive team as an Onsite Care Advocate. In this role, you would visit our affiliated facilities to ensure our valued members are receiving the best quality of care/services. This is a challenging position that offers a chance to showcase your talents while enjoying a great team atmosphere and a chance to make a true impact on the healthcare industry.

 

Primary responsibilities include:
Conducting onsite clinical chart assessments to evaluate and identify delays in care
Facilitating discharge planning
Arranging alternative care services
Working with treating physicians and other healthcare professionals to better understand care plans
Recording the daily census for each facility and reviewing patient charts
Helping to determine the need for care intervention 



The chosen candidate must be an RN with a current, unrestricted license in the state of Florida and ample clinical experience. Prior discharge planning and/or chart review experience is highly preferred, along with knowledge of the managed care industry. A valid driver’s license will be needed (to visit various facilities).
