Primary Care Coordinator, RN

Long Term Care Group, Inc. (LTCG) is the largest long-term care administrator in the country.  We provide services to some of the largest insurance companies in the nation. LTCG offers a complete platform of long-term care insurance services, such as underwriting, care management, commission payments and much more.

 

We are in search of a RN Primary Care Coordinator for coordinating medical services, supporting the family/patient needs, educating the patient about their diseases and ways to halt progression for each patient admitted into one of the case or disease management programs.  This position will assess, plan implement and evaluate the patient care plan in collaboration with the patient and/or family, the patients PCP and other health care professionals.

 

Primary Responsibilities:

?  Documentation of case findings and authorizing medical services.

?  Completing extensive health assessment for each patient admitted into case and disease management.

?  Development of care plans for each patient.  The care plan will be revised and updated each time there is a change in the needs of the patient.  Also responsible for stratification of the patient into the level of care indicated based on the care plan.

?  Coordinating medical equipment and/or services.

 

Qualifications:

?  Valid and Unrestricted RN license in MN.

?  Minimum 4 years clinical experience, preferable with older adult population in a community setting or long-term care facility required.

?  Established critical thinking; problem solving and decision making skills

?  Ability to prioritize and effectively handle multiple job tasks.

?  Ability to independently manage and lead projects to a timely completion with minimal supervision.

?  Strong organizational skills.

?  Detail oriented.

?  Working knowledge of Windows.

?  4 year degree preferred.
