Responsibilities: The Home Health Nurse applies the practice of nursing to a patient with a health deficiency in the client's residence. The primary responsibilities include initiation and evaluation of resources and interventions needed to promote the patient's optimal level of well being. An Admit/Recert RN is responsible for planning and delivering patient care in cooperation with the patient and/or significant others, and through collaboration, coordination and evaluation of other formal and informal services, required to assist the patient to maximum self-care responsibility. Requirements: Current license in the State of Illinois as a Registered Professional Nurse; valid Illinois Driver's License. One to three years home care experience and medical-surgical experience; access to auto during work shift; current auto liability insurance; computer literate; interpersonal skills related to leadership and flexibility.
