RN-Care Coordinator

POSITION PURPOSE:

 

A collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates options, services and alternative levels of care to meet an individual?s needs.  By assuming a leadership role with the interdisciplinary team, the Case Manager promotes appropriate utilization, high quality care and cost effective outcomes.

 

 

NATURE AND SCOPE:

 

The Case Manager has the responsibility to promote case management activities through

the health continuum, beginning in the acute setting.  Case management begins with the

assessment of pre-morbid health status, current medical condition and post-acute needs

 

Utilizing an interdisciplinary team approach, the acute case manager acts as a consultant and educator on matters referring to alternative levels of care, managed care issues, and community resources. In collaboration, to provide optimal patient care through, assessment, planning, implementation, and evaluation of neonatal, pediatric, adolescent, adult, and geriatric patients and families.

 

The acute case manager assures a smooth transition to the next level of care

at the time of discharge.

 

It is common to encounter potential hazards in the healthcare environment. Some of these hazards could include, but are not limited to: Radiation, Toxic Chemicals, Biological Hazards, Heat, Noise, Dust and Stress. Washoe Health Systems has a Safety Management Program in place addressing these issues.

 

MINIMUM QUALIFICATIONS:

 

Education:      Graduate from an accredited school of nursing

(BSN Preferred)

      Ability to read and write English.

 

Experience:     Minimum 2 years acute hospital experience, required.

      Rehabilitation or SNF experience, preferred.

      Home health experience, preferred.

 

License(s)/Certification: Current Nevada RN license required

Certification in Case Management (CCM), Professional Utilization Reviewer (CPUR), or Managed

      Care (NMCC), preferred.

      Current CPR certification by AHA standards, required.
