Yoh Health Care has an exciting opportunity for a Nurse Case Manager at a healthcare organization company in Center City Philadelphia.

Description:

We are currently seeking a Registered Nurse to work in the capacity of Nurse Case Manager. This is a Full-Time, Benefited position with great opportunity for growth.

Responsibilities:

* Case Management
* Manage patient referral process
* Effectively communicate with providers, staff physicians, patients and family members
* Complete clinical assessments and care plans for patients
* Education of diagnosis, disease, treatment and insurance issues
* Rate negotiation with providers in and out of network

 

Requirements:

 * Must be a PA Licensed RN
 * Previous health insurance, TPA, managed care experience 
 * Computer skills a must
 * Past case management experience preferred
