RN Case Manager - Part-Time

This part-time position in our Case Management department will entail conducting onsite and or telephonic assessment, coordination of appropriate plan of care, working with the member and or member's family, health plan representatives and other healthcare professionals to coordinate care to meet the patient's overall care requirements. Strong candidates will possess excellent communication customer service, and negotiation skills and excel at working with physicians, patients/families and collaborating on best practice. Requirements: Current unrestricted CA RN License, RN essential, with BSN preferred. A minimum of 3-5 years recent acute clinical experience required, with managed care experience preferred, preferably in an HMO or IPA setting.
