RN Case Manager....Registered Nurse Case Manager

RN Case Manager ? Telephonic position, Monday ? Friday, 9-5, conveniently located in Queens with parking!!!
This position works with Members and Providers to target the reduction of complications to achieve better care and patient outcomes. Specifically, the RN Case Manager uses knowledge and expertise of The Company?s benefits and community resources (par & non par) to coordinate services appropriate for the particular member; assesses the Member?s health status and the appropriateness of the indicated treatment plan; acts as the Member?s advocate as necessary; and provides education to patients, families, physicians, caregivers and facilities as appropriate. In addition, the Case Manager coordinates utilization areas, including concurrent review and discharge planning for appropriate referrals; and identifies quality issues and refers them for appropriate review.
Educational Requirements: NY State Registered Nurse License required.
Work Experience: 3-5 years, including clinical and case management experience.
Salary range: $65k-$70k.
