RN Dual Eligible-Care Management Coordinators

We are currently seeking several nurses as telephonic Care Management Coordinators to collaborate on Care Plan development and coordination with physicians and members by screening, triage, assessing, planning, facilitating, monitoring, and advocating for the member on an individual basis.  The successful candidate will facilitate communication between the patient, family, and all members of the health care team.  While ensuring that high quality, cost-effective services are delivered; the Coordinator will serve as an educator and link between the patient, providers, and the Plan.  Goals are to maximize effective use of resources, ensure timely and effective service, promote optimal level of wellness, and assist members in self-directed care.

 

Qualified candidates will be a RN with an active PA license and possess a minimum of three years recent clinical experience.  Previous computer experience is required.  Utilization review and discharge planning experience is preferred.  In addition, experience in planning, arranging, or providing post-acute care services is preferred.
