RN II - TEAM LEADER (10991)

Primary Duties and Responsibilities
Ensures quality and safe delivery of services to patients according to care plan. Uses equipment and supplies safely, effectively, and efficiently.
Provide leadership to ensure effective, efficient operations, optimizing performance and continually improving quality of care in a vastly changing health care environment.
Consistently utilizes a holistic approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patients served in assigned service area.
Provides patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues.
Maintains accurate and timely documentation of clinical records and day sheets.
Integrate and coordinate the care of the at-risk population across the healthcare continuum making assessments, planning interventions working closely with providers and overall patient care team.
Facilitate the interdisciplinary plan of care to meet multiple service needs as well as promote continuity through elimination of fragmentation of care/service.
Assist with the coordination of orientation and training programs for new staff members.
Assist with additional cases that are unable to be met by current staff.
Assist with performance evaluations, coaching, counseling and disciplinary action.
Assist with the overseeing the day to day management of assigned service line including the coordination of operations, establishment of work schedules, review of staffing needs and the supervision and training of staff in good patient relations and policies and procedures as needed.
Serve as patient advocate, educator and as central source of communication for the patient and their support systems, and referral source.
Responsible for coordinating the patient care team in providing quality, cost effective patient care in the ambulatory care setting.
Monitor the patient care process and collect data to evaluate and improve care collaborating with providers and other health care workers to provide comprehensive health assessment, treatment and follow up evaluation to include risk appraisal, health promotion and clinical research.
Maintain awareness of state and national health care trends and proactively responds with initiatives to enhance provision of care and ensure efficient resource utilization.
Schedule referral appointments and distribute patient educational information as directed by provider.
Responsible for accurate documentation (ie: care plan, care and utilization of resources to achieve quality outcomes, conferences, discharges, billing, coding etc.)
Maintain regular and consistent attendance at work.
Behave in a manner consistent with Mission, Vision, Values and Expectations for Excellence.
Maintain compliance with OSHA, Accreditation Standards, and Risk Management guidelines.
Maintain compliance with Personnel policies and procedures.
Behave in a manner consistent with all Corporate compliance policies and procedures.
Perform other duties as requested by the Clinical Manager to facilitate smooth and effective operations of the organization.

Qualifications
QUALIFICATIONS REQUIRED:
Graduate of State Board approved program for Registered Nurses.
Current license to practice nursing in the State of Iowa for Registered Nurse
Current Basic Life Support for the Healthcare Provider Certification or equivalent
Mandatory Reporter certification.
Strong interpersonal skills.
Ability to work as a team member.
Ability to understand and apply guidelines, policies and procedures.
Licensed driver in the State of Iowa with automobile insurance in accordance with state and/or organizational requirements
CPR: Maintain a valid Healthcare Provider Card with Re-Certification

QUALIFICATIONS PREFERRED:
Masters education in health related field.
BSN or BS in health related field.
Previous home care experience.
