RN MMQ Coordinator M-F 9a-4:30p.

RESPONSIBLITIES OF THE MMQ CO-ORDINATOR

1. Audit all Medicaid records weekly, using the comprehensive audit tool. Forward all discrepancies to DNS and assist nursing staff to evaluate the resident to determine the resident?s actual status and functional level. Ensure that all incorrect documentation is corrected using the proper record correction procedures. (Note: If progress note or ADL is incorrect the person who made the incorrect entry must make the correction with in 15 days. Info corrections may be made within 7 days of MDS completion date. If significant corrections are needed after the 7-day period: a new MDS must be done. The reason for the assessment (Section A8a) would be #4 (Significant correction of a prior MDS)

2. Review and update all resident profiles during every month. This requires consultation with Licensed and C.N.A. staff on all 3 shifts as well as visual inspection of the ADL status. When making changes, be sure they are communicated to other staff so all documentation will agree. Please use Status Change Notice and hand out at morning report.

3. Ensure that all necessary ADL and Flow Sheets are in place every month.

4. Review all treatments monthly and ensure that proper documentation is in place and Dr?s orders are present. Monitor for use of prescription ointments and ensure Dr?s orders include Dermatological diagnosis. During documentation month, photocopy all Dr orders and check charting daily.

5. Review Skilled Observation monthly and ensure proper documentation is in place and that initial episodes within 60 days are properly documented. Photocopy all documentation of the initial episode. 6. Review weekly skin assessments of all residents before the 15th of the documentation month.

7. Monitor daily 24 hour report and morning report for potential treatments or skilled observations. Utilize morning report as a vehicle for teaching the nursing management staff.

8. Monitor all Nursing Summaries as they are completed. Require all licensed staff to leave summaries in your box as they are done. This will allow you to work with the Licensed
Staff to teach correct way to write a summary. DO NOT LEAVE NOTES. This must be a hands on process in order to be effective.

9. During documentation month, arrange to be PRESENT during the first three days of ADL documentation on all shifts. This will enable you to monitor documentation and assist c.n.a.?s to understand codes so they can choose the code that accurately reflects the actual status and care of the residents. This also gives you an opportunity to be sure the profile accurately reflects the status and care. It also give you an opportunity for in servicing c.n.a.?s on correct use of the Resident Profile and assures they will understand the profile. You will be able to explain the language on the profile when there is an inconsistency so that C.N.A. can decide (1) if the profile was incorrect, (2) if their understanding of the profile was incorrect or (3) if there was a change in resident status. This process will help C.N.A. to not feel they must chart according to profile when the observed status of resident is not reflected.

10. Monitor all ADL Flow Sheet daily for the remainder of the month and 2x a week during documentation month. When inconsistencies are noted you must meet with the staff involved. DO NOT LEAVE NOTES. This must be a hands on process to be effective.

11. Monitor all accidents reports for potential treatments & Skilled Observations

12. Complete all admission and conversion MMQ?s

13. Conduct ongoing in-service of all nursing staff.

14. Conduct orientation in-service of all new employees and give handouts as follows:
