This position entails:
?Conferring with physicians in
developing the initial plan of
treatment.
?Coordinating appropriate care
through various clinical staff.
?Supervision of on going services.
?Coordinating the clinical and
financial components of care
management.
?Promoting quality, cost effective
patient outcomes through
communication and available
resources.
?Providing admission, case management and follow-up skilled
nursing visits.
