RN/Team Manager

We are seeking an RN/Team Manager for our Chicago South program.



The RN/Team Manager is the leader of the patient care team whose function is to supervise, evaluate and coordinate the various component members of the interdisciplinary team. Responsibilities include continuity of care from admissions to discharge or transfer to bereavement. The Team Manager also serves as a patient advocate and coordinator for other social services and health care providers in the community.



Qualifications include an RN IL licesnce and previous hospice experience and a minimum of two years management experience in a healthcare organization.
