RN/Team Manager We are seeking an RN /Team Manager for our Inland Empire Program located in San Bernardino, CA.

 

The RN/Team Manager is the leader of the Patient Care Team whose functions include:
Supervising, evaluating and coordinating the various members of the interdisciplinary team.
Providing continuity of care from admissions to discharge or transfer to bereavement.
Ensure that services are rendered as integrated components of the care plan.
Monitor the type and level of team services provided to assure appropriateness and responsiveness to patient and family needs and expectations.
Chair team meetings, assure updates and revisions of the plan of care.
Acts as a resource and mentor to staff.
reviews and approves payroll, mileage and expenses.
Performs chart reviews and annual program review.
Analyzes customer service issues on their team to identify causes and works with individual team members to improve performance
The Team Manager also serves as a patient advocate and coordinator for other social services and health care providers in the community.

Qualifications includes a CA state RN license, previous home health or hospice experience with a minimum of two years management/supervisory experience in a health care organization.
