Registered Nurse - Clinical Nurse IV

To provide proficient nursing care for an individual or group of patients with less predictable problems and to provide leadership in the implementation of high quality nursing care in the Urology Department.

1. Dependent upon area of assignment performs all, or a combination of the following:
2. Delivers expert nursing nursing care based on in-depth knowledge and expertise by:
3. Assess factors of the patient"&tm;s past and present physical, psychosocial, and cultural
4. baseline, health problems, and external support systems which may impact on his/her ability to cope with current health situation.
5. Define a nursing problem list, based on assessment of the patient, that reflects the
6. Consideration of the medical diagnosis and the patient"&tm;s problems,
7. Define realistic measurable goals related to each problem.
8. Develop a nursing plan which is directed toward the optimal resolution of nursing diagnoses and prevention of future problems.
9. Evaluates and document a patient"&tm;s progress toward goals.
10. Collaborates with the appropriate health care professionals in implementing a comprehensive plan of care.
11. Provide an individualized plan of nursing care which assists the patient in maximizing participation in his/her care.
12. Collaborate with physician to revise the nursing plan of care and/ or goals as necessary.
13. Implement appropriate patient education programs to meet individual health care
14. needs.
15. Assist physicians with day to day patient care
16. Taking histories and assessing reason for visit.
17. Assist with procedures and treatments
18. Provide patient teaching when necessary
19. Records nursing care using appropriate documentation.
20. Demonstrates administrative and clinical leadership skills by:
21. Monitoring and evaluating the practice of primary nursing according to standards..
22. Anticipating patient care or staff problems that may arise and initiating preventative measures.
23. Determining numbers and types of personnel needed and taking action to obtain staffing.
24. Assessing patient needs and personnel capabilities in determining assignments.
25. Assisting less experience staff in expanding their knowledge and skills.
26. Evaluating working relationships of staff members and providing counseling to improve communication and interaction between staff and team members.
27. Planning and implementing formal unit programs including orientation, in-service, patient education, quality assurance and systems of patient care management.
28. Developing, evaluating, and revising standards, policies and procedures within area of practice.
29. Counseling and evaluating staff under direction of Clinical Nurse 5, Chairman, Clinical Manager, and/or Practice Group Administrator.
30. Serving as a role model in the area of professional and ethical behavior.


31. Precepting and orienting staff to policy, procedure and clinical situations under direction of Clinical Nurse 5, Chairman, Clinical Manager, and /or Practice Group Administrator.
32. Maintains accountability for delivery of nursing care and for aspects of care delegated to others.
33. Adheres to ethical and legal standards of the hospital and the profession.
34. Serve as a content specialist to develop and maintain a high quality of care for a specific patient population.
35. Participates in identification of research questions and collection of data.
36. Assist with telephone triage when necessary as a back up staff member
a. To give advice to patients via telephone and in office
b. To assess the needs of patients via telephone
c. To handle calls in order of priority
d. To make appointments whenever necessary
e. To assist with referrals from outside physicians
f. To re direct calls when appropriate
g. To work directly with other support staff to handle problems
37. Maintains professional expertise by earning contacts hours through review of current Nursing literature and/or attending conferences.
38. Participates in staff meetings.
39. Develop goals in consultation with nursing manager for professional growth and development.
40. Participates in unit level and department committees.

Required Skills:
Education: Current D.C . Licensure as a registered nurse or evidence of application for licensure in D.C. and current out-of-state is necessary. Bachelor"&tm;s degree in nursing- OR- Diploma or associate degree in nursing.

Experience: At least two years of relevant clinical experience is necessary.
