Registered Nurse - Hospitalist Clinical Care Coordinator - Milwaukee, WI

Cogent Healthcare, a national leading hospitalist company, seeks a Registered Nurse to join our hospitalist team in Milwaukee, WI as a Clinical Care Coordinator. This is a unique opportunity for the right RN to make the transition from the bedside to a more administrative role while still maintaining a clinical focus.

 

The hospitalist nurse coordinator collaborates with the hospitalist care team to coordinate and manage the process of patient care from the point of admission to the hospital, through discharge and hand-off to a community care provider.
