Registered Nurse - Resident Care Coordinator
Position: RESIDENT CARE COORDINATOR
Reports to: ADMINISTRATOR
Summary:
The Resident Care Coordinator is responsible for providing oversight of the resident health care service needs. The Resident Care Coordinator is also responsible for the overall medication delivery system and the coordination of in-service training of the Health Services department.
Duties and Responsibilities:
1. Adheres to and conveys the philosophy and mission of Senenet, Inc.
2. Is aware and knowledgeable of the state rules and regulations as well as the
regulations of the state's nursing commission.
3. Maintains adequate attendance, notifies supervisor if late or unable to work in accordance with
personnel policies, follows work schedule.
4. Develops the monthly health services staffing schedule and participates in after hours on call
related to health services department in accordance with scheduling guidelines. Will share in responsibilities of covering open shifts in accordance with the scheduling guidelines and state specific regulations
5. Participates in the recruitment, performance evaluation and disciplinary action of health
services department staff.
6. Keeps resident information confidential, respects resident?s rights, respects privacy and the right
to self-determination by the residents.
7. Abides by established policies and procedures of the facility. Looks for ways to improve facility
function.
8. Conducts Health Services related audits in accordance with community QA program.
9. Facilitates the overall coordination of care for residents and provides oversight for the health
services department staff in accordance to the Health Service Manual and through participation in
the following:
a. Move In Process
? Participates in the pre-move in assessment, review of move in paperwork and initial service plan development in accordance with current policy and procedure
b. Assessment and Service Plan
? Participates in routine review of resident service plans to identify needs for additional assessment, health monitoring, or other coordination of care needs.
? Participates in service plan review process either by attendance or through review for all residents.
? Coordinates residents health care in collaboration with outside health care agencies ie pharmacy, physician, home health, hospice etc.
c. Staff Training and Education
? Coordinates and participates in initial job duty specific orientation and training conducted for all health services staff in accordance with facility policy.
? Verifies completion of job duty specific orientation and training through routine review of the training documents as per current policy and procedure.
? Coordinates continuing training and education in accordance with state and facility policy and as identified through routine auditing of health services systems.
d. Delegation (as applicable by state specific regulations and to be conducted
only by an RN as per state board of nursing regulations)
? Provides RN delegation in accordance to state regulations and facility policy.
? Coordinates delegation needs with Administrator for new residents
? Will be available after hours or when not in the community for issues related to current residents? delegation needs.
? May be required to come into the facility or conduct an offsite assessment after hours or when not in community.
e. Infection Control
? Maintains staff TB testing and Hepatitis B vaccinations in accordance with state regulations and facility policy.
? Maintains compliance with facility policies on Immunizations, Infection Control and Infectious Disease Management.
f. Quality Assurance
? Completes routine auditing of all health services systems in accordance to Health Services Review Schedule and all other associated QA tools within.
10. Reviews and responds to health services related audits.
11. Participates in other health services related duties as assigned by Administrator.
Job Qualifications:Demonstrates knowledge and utilization of basic management skills.Requires minimal supervision, is a self-starter and is flexible.Organizes and utilizes time appropriately, sets priorities and accomplishes assigned
tasks.Displays mature behavior in attitude and speech and action and demonstrates
consistently pleasant demeanor, tone of voice, supports the philosophy and approach
used by the facility.Works cooperatively with supervisors and co-workers.Is accurate, dependable, neat and thorough in assigned tasks.Is effective in both verbal and oral communication.
Other Requirements:
Previous experience working with the elderly population is desirable. An unencumbered nursing license issued by the state of Oregon is necessary. Maintain CPR certification.
Universal Precautions:
Exposure to blood/bodily fluids is likely.
Physical Requirements:
Has ability to do occasional heavy lifting (up to 40 pounds).
Has the ability to stand for long periods of time on feet and ambulate quickly.
