Registered Nurses - San Francisco

Cogent Healthcare, the leading national hospitalist company, seeks two Registered Nurses w/acute care experience to join our new hospitalist team as Clinical Care Coordinators (CCC) in San Francisco, CA.RNs must have an understanding of hospital environment and case management principles.  The CCC collaborates with the hospitalist team to coordinate and manage the process of patient care from the point of admission to the hospital, through discharge and hand-off to a community care provider.

 

CCC position requires a California RN license with a minimum of 1 year inpatient adult acute care, case management and utilization management experience.  Well-developed communication skills.
