Transplant Coordinator-Bone Marrow
**THIS POSITION WILL BE LOCATED IN LOS ANGELES**
The Transplant Coordinator position provides for ongoing development and implementation of a national case management program to coordinate the care of members referred to medical centers for transplant-related BMT care.
The responsibilities for this position include developing and implementing case management programs for patients awaiting transplantation. Follow policies and procedures specific to the transplant program operations. May recommend new, or changes to, existing policies and procedures. Review referrals for transplant using specific patient selection criteria. Manage all phases (pre-, event, post-transplant) of care for patients with or awaiting transplantation. Facilitate transportation and housing arrangements. Consults with transplant team members regarding appropriate treatment plans. Coordinates the interdisciplinary approach to providing continuity of care, including utilization management, transfer coordination, discharge planning, concurrent and retrospective review. On behalf of the members home region, processes authorization for transplant related care.
Acts as transplant liaison between Kaiser Permanente and contracted transplant centers of excellence. Conducts regular weekly meetings with center of excellence transplant coordinators. Establishes and maintains effective partnerships between Kaiser Permanente and contracted transplant centers with the overall goal of providing total quality management for Kaiser Permanente members in the most effective manner. Identifies and recommends opportunities for cost savings. Assists in the evaluation of transplant contract rates, terms and strategy, and advises contracting staff and senior management of recommended changes to transplant contracts as requested. Maintains up-to-date knowledge of changing technology related to transplantation services and KP benefit and policy changes which impact administration of transplantation services.
Develop and update educational materials for the KP transplant program, including, but not limited to, information for members, families, and physicians. Participates in NTN educational opportunities, projects, and ongoing activities with NTN committees. Ensure that accurate records are maintained of the care associated with each transplant patient. This includes keeping and documenting appropriate nurses notes electronically. Maintaining an up-to-date directory of referring physicians, claims processing staff, and center of excellence contacts.
The requirements for this position include: Current RN license required. Oncology experience required. Bachelors degree, or equivalent in nursing or health related field. Masters degree preferred. Graduate of an accredited school of nursing. The incumbent must possess demonstrated ability to work effectively across medical specialties, communities, departments and regions with both physicians and administrators. . Significant (usually more than 5 years) acute care and critical care nursing experience. Demonstrated skills in customer service, program development, problem solving, utilization management, and discharge planning. Knowledge of UR/QA standards, and current technology in transplant care.
