Transplant Coordinators

Openings for transplant coordinators, managers, and administrators throughout the USA at reputable medical centers and OPO's. Needs in liver, kidney, kidney/pancreas, lung, and heart.

 

General responsibilities for coordinators include: Independently provides and coordinates care provided to patients within a clinical specialty. Serves as a fully effective and participative member of the multidisciplinary patient care services team. Demonstrates and models the full range of nursing professional practice in both care provided and in participation in development of strategies to improve patient outcomes, satisfaction with service and cost effectiveness at the unit level. Is accountable to patients and families for the processes and outcomes of care during an episode of care. Is accountable to peers for the advancement of group practice on the unit, and to the nurse manager for the totality of work performance. Establish and maintain professional relationships through open communication with Physicians, Nurse Practitioners, and other Coordinators on team, as well as medical staff within hospital to provide a continuity of care for the transplant candidate. Provide education to potential transplant patients to ensure an understanding of the transplant process and how it relates to their situation.

 

General responsibilities for NP's include:Performs comprehensive physical assessment of patients. Elicits a complete history from patient and/or family, including chief complaint, history of present illness, past medical history, family and social history, and review of systems. Performs a complete physical examination using the skills of inspection, auscultation, percussion and palpation.  Follows this procedure with all new patients and on inpatient admissions of patients under the care of the Nurse Practitioner.  Appropriate history taking and physical examination are also done for each patient visit.  The findings of history and physical examination are presented to and reviewed with the responsible physician.  Sees and examines patients on the inpatient units daily on rounds with the attending physician, where applicable. Establishes medical diagnosis for common short-term or chronic stable health problems. 


Requirements:

1-2 years experience within transplant services preferred. 3-5 years experience within ICU if no transplant experience. Bachelor's degree preferred. Certification preferred.
