Dean Health Plan is committed to providing a high level of service to members and health care providers.  We are seeking a nursing professional who is self-directed and enjoys a collaborative and supportive work environment.  The position works with members, providers, DHP staff and medical directors to facilitate high-quality, cost effective care for members, including facility-based outpatient services.  This position monitors and analyzes clinical data and identifies members needing case or disease management interventions.

 

Minimum requirements include: Graduate of an accredited school of nursing (RN preferred) and licensed to practice in the state of Wisconsin.  A minimum of 5 years of active nursing practice required.  Demonstrated ability to work effectively and proactively with members and providers to facilitate optimal care for the member and to engage the member in managing their healthcare needs.  Excellent analytical, clinical, communication and organizational skills and the ability to work well with and train others.  Must demonstrate a broad range of experience in the clinical nursing and the ability to lead and recommend changes/improvements.  Knowledge of computers and demonstrated ability to use various programs such as Word, Excel, Outlook highly preferred.  Requires a valid driver's license and personal, reliable vehicle for work related travel.
