Assistant Case Manager
About the Company In 1975, the City of Garden Grove initiated a citywide program providing services that targeted the senior population. In 1987 this program separated from the City of Garden Grove, became a non-profit 501(c)(3) organization known as Senior Meals and Services, Inc. Sixteen years later, Senior Meals and Services, Inc. is still serving the City of Garden Grove, and also continues to serve the neighboring communities of Cypress, Los Alamitos, Midway City, Rossmoor, Seal Beach, Stanton, Westminster, and Costa Mesa. Senior Meals and Services, Inc. is proud of successfully providing services, for over 29 years to the seniors residing in these communities. We are passionate in our effort of fulfilling our Mission. Throughout the over 29 years that Senior Meals and Services, Inc. has been providing services to seniors, over 25,000 different seniors have been served; there has not been 1 day when meals and other critical services have not been available for elderly participants in need. During FY 2005-06, Senior Meals and Services, Inc. served over 350,000 meals. We are extremely fortunate to have remarkable community and volunteer support. Last year over 750-950 volunteers donated in excess of 20,000 hours of their time. Our services can frequently mean the difference between a frail, homebound senior being able to live comfortably in their own home or being placed in an institution. Studies have shown that maintaining seniors in their own home is significantly less expensive than caring for the same senior in a 24-hour care institutional facility. Not surprisingly, the seniors' mental and physical health oftentimes remains more stable and may even improve, because seniors residing in their own homes function in a familiar environment where they feel comfortable, safe and independent. About the Opportunity The culture is truly collaborative. Everyone's input is valued. You'll be surrounded by people who are passionate about what they do. Conduct assessments with homebound seniors to determine eligibility. Maintain client records and files. Refer clients to the Office on Aging Information and Assistance service or the SMS Senior Case Manager, as needed. Assist in compiling and completing statistical reports. Monitor, document and counsel HDM Site Managers in regards to delivery routes, red-tagged clients and volunteer scheduling and recruitment. Assist the HDM site coverage and delivery as necessary. Community outreach to increase general awareness of the SMS programs and to target those underserved populations. Represent the Agency effectively at County and other meetings. Maintain professional standards in client and business related activities. To ensure that clients are best served daily, the position may be asked to assist in other areas of the organization as the need arises (i.e. delivery of meals, substitute at sites, assist in special events).

About the Community Orange County has approximately 400,000 older adults, which reflect about 8% of California's senior population. According to the U.S. Census 2000, this number is expected to double in the next twenty years. In our commitment to meet this increasing demand, SMS continues to develop ways to meet these needs without ever having to turn away a frail and isolated senior trying to maintain an independent, safe, healthy, and dignified lifestyle. We pledge to promote and enhance our vital volunteer programs that provide socialization. Without the unselfish efforts of our volunteer base and funding partners, SMS would not be able to provide daily care for our 500 homebound seniors. With the Federal, State, and local Budgets tightening, funding for programs promoting healthy alternatives to institutionalization is now more vital than ever.
