?  Initiate timely client assessment for case management services.

?  Develop and implement client friendly care plans.

?  Provide case management, education and assistance in the overall coordination of services.

?  Offer counseling, guidance and respite assistance to caregivers.

?  Promote awareness of social programs; assist clients/caregivers in accessing community services.

?  Maintain current resource list of appropriate community services for client referral.

?  Document case intakes, adherence to the care plan and follow-up visits.

?  Compile and report case load statistics in a concise, timely manner.

?  Other duties as assigned by supervisor
