CASE MANAGEMENT SUPERVISOR

 

The Human Services Council is seeking qualified applicants for the position of Case Management Supervisor for the Southwest

Washington Agency on Aging program. The individual selected for this position will be responsible to supervise and manage Case Management services and staff including development and coordination of service delivery and promotion of public access to services for seniors and adults with disabilities receiving in-home and community-based long-term care (LTC) services. Minimum of a Bachelors degree in a social science field plus four years experience in human services delivery, preferably case management and two years experience supervising direct service professional staff is required. Degree verification required.

 

The Human Services Council is a non-profit corporation established in 1957, governed by a volunteer Board of Directors. Human Services Council's mission is to improve peoples' lives by connecting them to opportunities, information and services that respond to their individual and family needs.

 

The Human Services Council operates the Southwest Washington Area Agency on Aging Case Management, Information and Assistance, Volunteer Center, VISTA, Retired Senior Volunteer Program, the Employment Transportation Brokerage, Medical Transportation Brokerage, Interpreter Services Brokerage, and the Long Term Care Ombudsman in Clark, Cowlitz, Wahkiakum, Skamania, and Klickitat Counties to accomplish this mission.  The Agency annual budget is comprised of federal, state, and local funds awarded through several agencies as well as United Way and other local voluntary support.  Approximately 80% of these funds are subcontracted out for aging and long-term care services throughout Southwest Washington for programs such as home delivered meals, transportation, and in-home care.

 

The beginning salary for this position is $3047.17.  The Human Services Council provides an excellent benefit package including vacation, sick and personal leave, medical, vision, dental and life insurance, employment assistance program and retirement contribution.

1). Describe your experience with service delivery including assessing the needs of older and disabled, vulnerable persons,

and what steps you took to assist them in arranging needed services.

2). Describe your experience developing and managing services within defined regulations and quality standards including any relevant experience with Medicaid and Non-Medicaid programs.

3).Describe your supervisory experience including hiring, employee coaching and development, and performance assessment and evaluation.
