Case Manager

PURPOSE OF POSITION

 

Participates and collaborates with the mental health provider in developing case plans to address social, economic, medical, and other issues that impact the client?s treatment.  Advocates on behalf of child and parent in community setting (e.g. schools, etc.).

 

ESSENTIAL FUNCTIONS of the CASE MANAGER

 Identify needed resources for child and family; provide referrals and direct linkages to resources.Develop and maintain community-based referrals to programs for children, youth, and their families.Interface with community agents (e.g. schools, DCFS, Probation Dept., etc.) on behalf of clients.Outreach to the community to promote awareness of the program.Provide client advocacy services to child and parents in community settings (e.g. IEP meetings, probation hearings, etc.)Provide feedback to treatment teams on case management issues.Supervise children in various program settings.Prepare reports as required.

? Adhere to agency policies and procedures and advances the goals of UAII in a manner that embodies the agency?s philosophy.Maintain strict confidentiality of client and personnel information.  Keep Director well informed regarding activities, pending issues and potential problems.

QUALIFICATIONS ? Required

 Demonstrated experience of one or more years in the provision of case management services in clinical or other human service setting, ideally mental health.Excellent knowledge and understanding of AB3632, probation, DCFS, and other child and family related laws, programs, etc.A self starter who is able to work independently and as a team member; who consistently demonstrates professionalism, courtesy, efficiency, excellent internal and external customer service, high ethical standards and behavior that contributes to harmonious relationships. Demonstrable ability to communicate effectively and professionally both verbally and in writing.Demonstrated ability or willingness to work with culturally diverse populations, youth, and professionals.Knowledge of and/or sensitivity to the needs of the American Indian community/culture.Willingness to learn new skills and participate in training sessions relevant to the position.Clear background check.Valid California driver?s license and automobile insurance.Availability to work evening and weekend hours as required.

 

QUALIFICATIONS ? Preferred

 Indian Self-Determination and Education Assistance Act (Public Law 93-638).Bachelor's degree in social work or closely related field from an accredited college or university.Experience working with the American Indian community.

 

PHYSICAL REQUIREMENTS

 Able to work at a computer for up to 6 hours per day.Able to perform the following tasks during the work day:  reading, writing, speaking, listening, typing, sitting, standing, walking and driving a car.Current negative/inactive Tuberculosis test/x-ray results on file.Negative results on pre-employment drug test.
