Case Manager

Since 1981, Morris Heights Health Center (MHHC) has had a record of distinction as the major provider of health care to Morris Heights and the surrounding areas. Born out of the local need for quality care in the area, MHHC is a non-profit organization funded by federal, and state, foundation grants, private and corporate donations.

 

A vacancy currently exists in our HIV Services department for a Case Manger - Community Follow-up Program.  Under the supervision of the Director of Case Management Services, this individual will provide comprehensive case management to HIV+ clients and their collaterals in the community setting or through office based visits as well as direct the day to day activities of a case management team.

 

The qualified candidate will possess a Bachelor's Degree in a related field with 3 or more years of experience working with HIV infected/affected clients.  Practicum in case management and/or HIV treatment adherence services desireable.
