Community Support Person-IMPACT Program

The primary responsibility of this person is to assist the client in achieving or maintaining self support & sufficiency, provide linkage and coordination of care, assist in rememdy of abuse, neglect or exploitation of youth who are unable to protect their own interests; to preserve, rehabilitate or reunite families, allow client to reside in a less restrictive setting. Will be assigned a maximum caseload of 10-15 adolescent males providing individual, group and family intervention. This position will also provide individual or family behavioral health counseling as needed or identified.

 

Mandatory Group Hours: Monday-Friday/3:00pm-7:00pm

 

*Home based hours will vary

 

Credentials:

-Minimum of a Master's Degree in Social Work or Counseling with appropriate licensure mandatory

-Strong understanding of needs of adolescent males and knowledge of gender specific interventions

-Demonstrated skills and ability to clearly and concisely communicate

-Possess working knowledge of community resources and work effectively with clients and families in conjunction with internal/external service providers

-Ability to manifest commitment to philosophy/standards and foster an environment that supports recovery for clients and families

-Value of cultural diversity, exhibit cultural competence and knowledge about the impact of culture and ethnicity on the treatment process

-Posses an understanding of family dynamics and commitment to family preservation principles

-Establish positive therapeutic relationships with clients and families in individual, group or community settings

-Demonstrated knowledge of group dynamics and the ability to promote group cohesion via use of family systems oriented therapeutic modalities and techniques

-Plan and implement individual or group interventions and therapeutic activities

-Ability to work as a member of a professional treatment team

-Possess knowledge of AOD, gang, school and community issues and the court system

 

Work Activities::

 

* Collaboratively works with all internal/external service providers and staff in an effort to ensure continuity of care and promote best practice service delivery

 

* Provide individual, group and family counseling per licensure standards

 

* Assists clients and families in developing social supports and gaining access to essential community resources

 

* Maintains all documentation and statistical reporting data per provision of services

 

* Maintains appropriate academic and professional credentials and licensure required by this position, by law or by agency policy

 

* Ensures the delivery of services is consistent with Rosemont Center's mission statement
