Counselor/Case Manager for Residential/ Case Management Mental Health Program

Human Development Services of Westchester has immediate openings for Counselor/Case Managers in our Case management and housing programs.

About HDSW:

Human Development Services of Westchester, Inc. (HDSW) mission is to empower and enrich the lives of those we serve by assisting them to make informed choices in determining and obtaining desired living, learning, working and social environments.

The mission is accomplished through the provision of housing, vocational services, case management, community support and mental health rehabilitation services. HDSW presently serves over 1,000 individuals and families throughout Westchester, many who are recovering from episodes of serious mental illness and are preparing to live independently, and others struggling to maintain their housing. All clients are low-income, and from various ethnic and racial backgrounds.

HDSW serves its clients and the community through a unique combination of inter-related programs, including a Residential Program, a Permanent Supported Housing Program, an Intensive/Supportive Case Management Program, and HOPE House. HOPE House is a psychosocial clubhouse, where people recovering from mental illness are encouraged to return to the mainstream by taking advantage of the many opportunities to pursue employment and education offered at the clubhouse. HDSW is also the designated Neighborhood Preservation Company (NPC) for the Village of Port Chester, with the goal of increasing and preserving affordable housing for village residents.

Description:

The Counselor/Case Manager functions as a member of the treatment team and provides Client advocacy. Works with clients in order to facilitate growth and development of the skills necessary for community living. Provides training in the activities of daily living through role modeling and use of restorative services, education and service planning in accordance with the principles of Psychiatric Rehabilitation. Works in either the Community Residence or Case Management Program.

Qualifications:

Master' or Bachelors degree in human services related field; or an Associates degree in human services field and three years experience in human services; or a high school diploma/GED and five years experience in human services. Must have own car and a valid NYS driver's license. Must have strong interpersonal skills, be organized and be computer literate. Bilingual preferred.
