Director of Social Services & Admitting
If your're a dynamic clinical professional with strong management skills, an exciting challenge awaits you at the Komanoff Center for Geriatric and Rehabilitative Medicine, a 200-bed hospital-based, skilled nursing facility for sub acute and long-term care.
The position oversees the admission, social work (including discharge planning) and medical records functions. It also develops and implements resident programs, care planning through an intensive interdisciplinary approach, assuring supportive family services and interacting with referral and community resources.
LCSW or MSW is required with AT LEAST 5 YEARS of related SNF management experience. Knowledge of SNF regulations, case management methods and third party payment arrangements necessary.
