Position Title: Family-Based Social Worker

Summary: The Family Based (FBS) Social Worker will be responsible for the implementation of family based treatment/services.  The FBS Social Worker should understand the Managed Care Model and have the ability to work with and coordinate treatment services with the Department of Social Services, Lead Agency, Communities for People and a wide range of community service providers.



 

Job Requirements:

 Complete assessments and develop treatment plans for parents and children.  The Treatment Plan will stress the families? strengths, as well as the areas that need to be addressed in order to promote family stabilization.

 Comply with the Treatment Model, which spans over a 12 week period:  Phase one ? 6-10 hours per week; Phase Two ? 5-7 hours per week; Phase Three ? 2-4 hours per week.

 Teach families to access community services; identify and assist families in gaining access to community resources.

 Adhere to the goal of the Treatment Plans, which is to stabilize families sufficiently to close the case with the Department of Social Services.

 

Measurable Goals:  Maintain a caseload of at least 8 cases at all times. Complete follow-up (under the direction of the Director of Family Services) with the Department to determine the number of families whose cases have been closed after receiving Family Based Services. Follow-up with families to determine (through surveys) whether or not they found the services helpful and what suggestions they would make to improve the service.

 

Qualifications:

 Master?s Degree in Social Work (license preferred) 2-4 years experience in social work Demonstrated familiarity with DSS policies/procedures and case practice issues, such as abuse, neglect and domestic violence and substance abuse Excellent written/oral presentation skills Ability to work in a multi-cultural and linguistically diverse community English/Spanish bilingual capacity is a plus Basic data entry and computer skills.
