Family Psych-Education Counselor

The Staten Island Assertive Community Treatment (ACT) program offers community-based treatment for individuals with a diagnosis of mental illness ? often with a concurrent substance abuse disorder ? and a history of non-compliance. The ultimate goal is to help clients attain stability in terms of their immediate needs and follow their treatment regimen, ultimately improving their psychiatric and medical stability and lessening their dependence on the inpatient hospital system.

FUNCTION

The Family Psych-Education Counselor assesses client needs, plans a course of action to address those needs, and monitors client progress. The Family Psych-Education Counselor must ensure that the clients and their supports are provided with appropriate, competent and professional information, referrals and advocacy services to expedite successful outcomes.

MINIMUM QUALIFICATIONS

Master?s degree and at least one year related experience. Excellent written and verbal communication skills and platform skills.

PRINCIPAL RESPONSIBILITIES

1.Develop and facilitate psych-educational workshops for clients and their family, friends and other collateral supports.
2.Develop plans to educate clients on social skills, activities of daily living skills, independent living skills, empowerment skills, interpersonal relationship skills, problem solving skills.
3.Develop service plan goals with the clients related to the development of independent skills.
4.Monitor and document client progress towards goals.
5.Report assessments and outcomes monthly.

EFFECT ON END RESULT

1.Clients served will transition successfully into mainstream society.
2.Client and their family, collateral supports etc. will develop skills to cope with difficult and unique situations.
3.The program will meet funder and agency requirements.
