Hospice Chaplain - Mesa, AZ



 

My client is a well established regional entity based in the West U.S. who has just started a new Hospice branch located in Mesa, Arizona.  We are in search of a Chaplain to support this new, state of the art, patient care organization.

 

My client is seeking a full-time Hospice Chaplain.  Prior experience as a Pastor or Chaplain is required, along with a Master's degree from an accredited seminary or school of theology. Four quarters of clinical pastoral education is preferred, however, one unit is required.Basic computer proficiency will be needed.  This position functions as a member of a caring and committed interdisciplinary team, with competitive salary and benefits. 

 

We are currently seeking a new Hospice Chaplain to visit patients and their families in the local Phoenix, AZ area to provide them with spiritual support and guidance. Primary responsibilities include: Completing a spiritual assessment of each patient and family Developing and updating a spiritual plan-of-care for each patient/family in collaboration with the Interdisciplinary Team Providing spiritual support and guidance for each patient/family in a timely manner and according to policies and procedures Participating in Interdisciplinary Team and Support Services meetings Making visits and/or phone contact with patients/families as determined by the plan- of-care Maintaining current and accurate patient records, documenting assessments, interventions, outcomes and patient/family education Maintaining relationships with the spiritual care community at large and assisting the patient/family in accessing resources as needed Assisting with Support Services on-call duties and referrals/admissions as needed Offering and coordinating memorial services and related activities for families, caregivers, contracted facilities and the community at large Providing spiritual support, guidance and education to the Interdisciplinary Team Participates in community education regarding spiritual matters

 

This is a full-time salaried position with competitive health care benefits, PTO, 401K, etc.  Salary is negotiable depending on experience.  The qualified person will be very knowledgeable and comfortable with basic computer software and PC functioning.
