Licensed Practical Nurse (LPN)

Major Functions:

 

Provides direct nursing care, as assigned, under the supervision of a registered professional nurse (RN) to clients in accordance with agency policies and procedures and current standards of Nursing Practice as defined in the New York State Nurse Practice Act.

 

Specific Duties and Responsibilities:

 

?  Picks up orders for prescribed treatments, medication and diagnostic testing.

 

?  Performs venipuncture and obtains other specimens for laboratory tests as ordered by the physician.

 

?  Monitors vital signs and performs physical examination, as indicated, to assess the health status of clients.

 

?  Communicates clients? health status to the Nursing Supervisor, treating physician/physician assistant in a timely manner.

 

?  Provides information to an RN for the development of a medical plan of care.

 

?  Provides client education and healthcare teaching relative to diet/nutrition, health screening, exercise, medication and physician ordered treatments.

 

?  Administers First Aid in accordance with Medical Director approved protocols.

 

?  Administers PPD?s and notes response to skin testing.

 

?  Coordinates physician referrals to external specialists/consultants by completing appropriate request and/or consult forms.

 

?  On a rotational basis, provides ?on call? 24-hour coverage, via beeper, during evenings and weekends as required.

 

?  Completes Medicaid coding sheets as per agency policy and procedures.

 

? Ensures the maintenance of accurate, complete, timely and high quality client records that comply with external regulatory standards and agency policy and procedure.

 

? Complies with 42 CFR confidentiality and HIPAA privacy and security regulations.

 

?  After review with the client and/or attending physician, files all feedback consult sheets and laboratory reports in the client record

 

?  Transcribes medication orders to the Medication Administration Record and documents all medication doses on the Medication Administration Record.

 

?  Transcribes physician orders for methadone, prepares and dispenses methadone to clients during designated clinic hours.

 

?  Monitors clients for adverse events or over/under sedation and reports findings to the Nurse Manager or Physician.

 

?  Reviews all medication orders and obtains renewal or discontinue orders from the physician and completes procedures to obtain medications from pharmacy vendor.

 

?  Maintains controlled substance records as required by regulatory agencies and agency policy.

 

?  Assists Nurse Manager with daily methadone inventory.

 

?  Communicates effectively, timely and professionally with medical staff, clients and clinical staff to foster a positive working environment and to provide quality care to clients.

 

?  Performs other duties as assigned by the Nurse Manager.

 

Requirements/Qualifications:

 

?  Graduate of an accredited program for licensed professional nursing.

 

?  Current New York State Licensure as a Licensed Professional Nurse.

 

?  A minimum of 2 years prior nursing experience, of which 1 year must include hospital, substance abuse or other behavioral healthcare organization nursing experience.

 

Preferred Skills/Knowledgeable/Abilities:

 

Experience in substance abuse treatment programs.

 

Excellent interpersonal skills, time management and the ability to work under pressure. 

 

Basic keyboard skills and computer literacy (Window based environment knowledge preferred).
