MSW Care Coordinator, Hospice

Providence Hospice of Seattle has an opportunity for an experienced MSW to coordinate and provide services to hospice patients, families and caregivers addressing a wide range of issues related to end-of-life care. Responsible for development and implementation of a plan of care in collaboration with the Hospice Interdisciplinary Team and promoting patient and family choice, assuring continuity of care, and enhancing the quality of life of dying patients.
Education & Experience:
1. Masters Degree from an accredited School of Social Work required
2. 1 year experience in a health care setting required; two years preferred
3. Prior experience in hospice care setting preferred.
4. Three years post master's work experience. One year prior experience in a hospice care setting may be substituted in lieu of 3 years post-master's work.


Required Skills:
Licenses & Certifications:
1. Current Washington State Counselor Registration or Social Work License or Certification.
2. Current Washington State Driver's license and reliable insured vehicle
3. Current CPR certification.
