CASE MANAGEMENT

Primary Duties and Responsibilities
Collaborates with health care providers, physicians, and families of complex patients to coordinate care through the health continuum. The case manager assists in transitioning through the continuum by following progress and/or compliance and responding to individual needs or barriers to optimal outcomes.

Qualifications
Masters Degree in Social Work, licensed to practice in the State of Iowa. Hospital work experience with discharge planning and case management is preferred.
