Medical Social Worker

The MEDICAL SOCIAL WORKER is a qualified member of the health care team who is responsible for: (1) assessment of psychosocial and emotional factors related to the illness and care needs of patients in their homes; (2) recording clinical and progress notes; and (3) coordinating other patient care activities/services to provide the highest quality of patient care in the geographic service area to which he/she is assigned. He/she works under the direct supervision of the designated supervisor.

Required Skills:
1. Master"&tm;s Degree from a school of social work accredited by the Council on Social Work Education.

2. Licensed to practice as a social worker in the state(s) in which he/she is required to practice, if the state has licensure practices.

3. A minimum of one year of social work experience in a health care setting required; previous home care experience preferred.

4. Ability to relate to and treat acutely ill/chronically ill patients/families and their particular problems.

5. Must possess strong patient assessment, patient management and documentation skills.

6. Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the patient.

7. Must possess the disposition and personality to work harmoniously with a wide variety of people and all levels of staff, and interact professionally as a member of a multi-disciplinary team.

8. Must possess a strong commitment to the goals, mission, and philosophy of the organization.

9. Ability to work flexible hours as required to meet identified patient needs.

10. Absolute integrity and the use of good judgment to handle the professional and confidential nature of assignments..

11. Ability to adapt to changing organizational needs.

12. Speaks, writes, reads, and comprehends English.
