Outreach Counselors

Position Summary:

 

Provide outreach to consumers via the phone and face to face meetings, in their apartments and community settings. Maintain regularly scheduled appointments with consumers. Works as a part of a supported housing program in providing guidance, education supervision and direct support to individuals with mental illness. Advocate for and support consumers in identifying and accessing the full range of community-based supports. Provide intervention and support strategies to assist consumers with crisis prevention, symptom management and the development of self-managed coping skills.

 

Qualifications:

 

Requires a bachelor's degree plus six months of related experience, or at least three years of related experience.  Car and valid driver's license is required.
