As part of the Telecare community, Stanislaus Homeless Outreach Program (SHOP) in Modesto provides assertive community support to mentally ill adults who are homeless or at risk of becoming homeless.  The goals of the program are to reduce homelessness, jail recidivism, substance use and psychiatric hospitalizations and increase quality of life through education, employment, or other constructive activity.

Personal Service Coordinators/Case Managers

As a Personal Service Coordinator you will be an integral part of an individual?s road to recovery. In this exciting and fulfilling role, you will provide culturally competent and client centered coaching, counseling, mentoring, instruction, training, feedback and positive reinforcement in areas such as financial management, housing, health, employment and relationships. In short, you will make every effort to empower individuals to improve their quality of everyday life.

 

The successful candidate will have one (1) year of psychiatric experience as well as a degree in social work, nursing, occupational, recreational, music, dance, drama or art therapy or vocational counseling or related work experience. Valid CA driver?s license is necessary. Must complete first aid and CPR training or obtain certification within four (4) weeks of hire. Background in vocational, housing, or dual diagnoses services preferred.
