Essential Duties and ResponsibilitiesDevelop individual support plans (ISP) and monitor the implementation of the ISP according to company policy. Revise ISP as necessary to ensure the programmatic and health and safety needs of the consumer are met in a timely and effective manner.
In group-home settings, the QMRP functions as facilitator for each consumer?s Person Centered Planning process. In Medicaid Waiver settings, the QMRP works cooperatively with the case manager throughout the person centered planning process.
Conduct programmatic reviews as outlined in the consumer?s ISP and in accordance with company policy and procedure to include review, analysis and proper filing of program documentation.
Interview each consumer on case load to determine whether the consumer has experienced or observed abuse or neglect; document interview results and immediately reports any allegations of abuse/neglect.
Implement area systems for routine contact with consumers and their family members to maintain high levels of consistency between consumer expectations of services and services delivered.
Work cooperatively with OM's and nurses to translate identified consumer program needs into direct-care staffing resource requirements.
Provide initial, consumer-specific training to each new program site direct-care staff to ensure new staff are trained in the individual needs and programs of each consumer.
Provide on going training and assessment to personal support assistants to ensure services being provided to consumers are consistent with goals and outcomes identified in a consumer?s ISP.
Participate, with Area Director and OM's, in assessing the competencies of direct-care staff, conducting performance reviews, and recommending disciplinary action of personal support assistants, nurses, and OM's as necessary.
Cooperate with other Area Directors and DSA, Inc. QMRP?s to develop and participate in an on-call schedule to assure any critical consumer issues are handled in a timely and effective manner.
Maintain consumer programmatic records in a manner consistent with HIPAA regulations, other Federal and State regulations and applicable DSA, Inc. policies and procedures.
Participate as an instructor in the agency Staff Development program.
Complete other duties as assigned.
Qualifications
Bachelor?s degree in psychology, social work, nursing or related human services area and a minimum of one year of work experience in developmental disabilities.
Ability to communicate effectively; both orally and in writing.
Ability to use common word-processing, spreadsheet and communications software programs within a Windows environment.
Satisfactory completion of a criminal background check as required by federal and/or state laws and any other post-offer pre-employment screenings as mandated by a governing agency or DSA, Inc. policy.
Must possess a valid Indiana driver?s license, a good driving record and a current automobile liability insurance policy.
