Registration Specialist - Mental Retardation

COMPANY OVERVIEW:

 

We are a community based agency that is the single point of contact for entry into the early intervention system. We provide service coordination for children from birth to their third birthday at no cost to families. All services are provided in the home or in community settings.

 

 

 POSITION SUMMARY: 

The Registration Specialist is responsible for the operations surrounding assessment of individuals for registration in the Mental Retardation system.

 

Additional responsibilities and guidelines associated with this job is directly related to upholding the principles of Self Determination by giving families and individuals accurate and reliable information regarding the Mental Retardation system and other system services as appropriate. Families and Individuals will also have an opportunity to make an informed choice of service provider.

 

 

DUTIES AND RESPONSIBILITIES:

 Respond to family and individual calls requesting services by meeting with the family at a location convenient to the individual and/or family.Gather pre-registration information as appropriate so that a determination may be made as to whether or not this individual is already in the MR system. If not, enter primary demographic information into HCSIS as required.Conduct intake meeting with individual and family. Gather information necessary to determine whether or not the individual is likely eligible for MR services. The Registration Specialist is responsible for determining whether or not an individual may be eligible for MR services. If there are any question regarding this, referral to a QMRP would be warranted.Assure that the individual and family receive accurate information regarding the various MR services/providers.Complete and have the family sign a PUNS form.Assist individual or family in the completion of application for MA, if appropriate. Assure that individual and family are apprised of their rights regarding appeals processes. These include county conference, mediation, Fair Hearing.Gather information to determine likelihood eligibility for waiver funded services. Update HCSIS screen as required.Complete a situational assessment based on information from the individual and/or family. Calculate an IER based on information received.Inform family of choices of ISC Units. Once individual/family chooses a Unit, assign a Unit by notifying and transferring information via HCSIS.Participate in Individual Planning Meeting Team meetings as determined. Intake/Registration Specialist will need to be able to speak to trends in referrals, requested services, as well as other types of requests as it may apply to determination of funds through initiatives or Vacancy Management activity.Work collaboratively with Quality Specialist to outline and relay necessary information in order to address uphold quality factors in relation to planning and service delivery. This includes reviewing and analysis of various reports from the county and state.Work collaboratively with Policy/Planning Specialist as necessary when county policy needs reviewed or improved in areas relevant to planning and service delivery.Implement policies, procedure, and criteria based on county, state, and/or federal regulation.Participate in on-going training regarding changes in the HCSIS system as it pertains to the roles related to intake, eligibility, and registration.

 

QUALIFICATIONS: 

1.  Must have a degree in social work or a related field, or equivalent combination of education and experience.

2.  Previous administrative experience desired.

3.  Experience in the field of mental retardation is recommended.

4.  Experience working with families is recommended.

5.  Must have excellent communication, organizational, problem solving and analytical knowledge, skills and abilities. 

6.  Must have excellent knowledge of MH/MR service delivery and systems theory, provider delivery systems Must have basic computer skills in Microsoft Word, Outlook, Excel, and inter-net research methods.

 

JOB REQUIREMENTS:

 

? Possess a reliable motor vehicle and a valid PA motor vehicle license.

 

? Have Acts 33 & 34 clearances.

 

?  Have ability to access family/individual home or other community location for visits and intake meetings
