Service Coordinator

Seeking hardworking, enthusiastic person to fill a one-half time position as a Services Coordinator for the Center for Independent Living of Central Nebraska in Grand Island. This person will provide services to persons with disabilities who are eligible for Medicaid but elect to particpate in "waiver services." Will coordinate all services to consumers under the Medicaid Waiver program. Bachelors degree required OR Registered Nurse licensed in Nebraska OR HHS Social Services Worker who has received case management and waiver training to implement and operate the waiver program for a min. of 5 yrs, AND at least 2 years experience in long-term care, gerontology, rehab or community health OR provided direct service to individuals with disabilities with case mangement preferred. Must have valid drivers license and dependable transportation.
