Social Work Care Coordinator I

POSITION PURPOSE:

 

The Social Work Care Coordinator is position is responsible under the supervision of the department leadership, for providing patients and families with emotional support, advocacy as appropriate, financial information, discharge planning and social work services.  This position also serves as a liaison between the medical staff and the patient/family and provides interventions designed to minimize patient stress and maximize coping skills to expedite recovery.

 

Care Coordination is a collaborative process that assesses, plans, implements, coordinates, monitors and evaluates options, services and alternative levels of care to meet an individual?s needs.  By assuming a leadership role within the interdisciplinary team, the Social Work Care Coordinator promotes appropriate utilization, high quality care and cost effective outcomes.

 

This position provides patient care services.

 

NATURE AND SCOPE:

 

This position provides psychosocial services to patients, families, and staff to assist in dealing with illness and hospitalization.  This position also provides educational information and referral services to community resources that may assist patients/families during and after hospitalization.

 

The incumbent must provide optimal patient care through assessment, planning, implementation, and evaluation of neonatal, child, adolescent, adult and geriatric patients and families.

 

The Social Work Care Coordinator has the responsibility to promote care coordination activities through the health continuum, beginning in the acute setting.  Care coordination begins with the

assessment of pre-morbid health status, current medical condition and post-acute needs. The Social Work Care Coordinator assures a smooth transition to the next level of care at the time of discharge.

 

It is common to encounter potential hazards in the healthcare environment. Some of these hazards could include, but are not limited to: Radiation, Toxic Chemicals, Biological Hazards, Heat, Noise, Dust and Stress. Washoe Health Systems has a Safety Management Program in place addressing these issues.

 

MINIMUM QUALIFICATIONS:

 

Education:      Ability to obtain and maintain State of Nevada Social Worker

      licensure.

 

Experience:  Requires one-year experience in the social service field.  Applicants

  with experience in health care settings preferred.

 

License(s)/Certification(s):  Requires valid Nevada license by the Nevada State Board of Examiners for Social Workers, or eligibility for licensure in Nevada.  (Please note that to qualify for the SW 1 position, the incumbent must hold a bachelor?s degree in social work or related field.)

   

Typing:   Working knowledge of basic computer programs.

      Keyboard knowledge.
