Social Worker Care Manager - Oakland County
Under the general supervision of the Regional Supervisor, uses person centered planning principles to assess the medical, functional, psychological, financial, and environmental needs of potential Community Care Services clients in order to develop client care plans and negotiate services required under the plan. Also responsible for ongoing monitoring and reassessment of clients and quality assurance activities.
Minimum requirement of Licensed Bachelor Social Worker. Minimum of 6 months of experience with psycho-social assessment or home care required. Three years of experience preferred. Experience with the target population preferred. Demonstrated problem solving ability and knowledge of long term care programs and eligibility requirements. Excellent organizational and interpersonal skills. Ability to work in a Community Based setting, independently, with little direct supervision of daily duties. Strong computer skills including minimum 25 wpm typing. Requires daily travel within a geographic region utilizing own reliable transportation.
