Social Worker II (Emergency Dept)

Overview : The purpose of this position is to develop, coordinate and provide social work services in the Emergency Department to patients and families who are faced with social, emotional and situational stressors precipitated by illness, injury, and/or disability . The goal is to aid in adaptation and empower the patient and the family to participate to the fullest of their abilities in the discharge planning process. This position recognizes the import of cultural diversity and provides developmentally appropriate care for the neonate, pediatric, adolescent, adult and geriatric population that it serves. It includes planning for the safe discharge and continuity of care, the ability to recognize and plan for the unique needs of all ages as well as the physically disabled, mentally ill, chronically ill and terminally ill patient.

Responsibilities :

1. Assesses the functional and psychological needs of the patient within the framework of his developmental stage, functional abilities, cultural milieu, and support network
2. Formulates a plan of intervention acceptable to the patient, family and medical team; proactively facilitates adjustment in the care plan or services when necessary to promote enhanced outcomes.
3. Documents all interventions in the patient medical record both timely and accurately including all elements of the discharge plan.
4. Provides intervention in the case of suspected abuse/neglect/exploitation, documentation and reporting when required. Assists patients/families with custody, consent and adoption issues.
5. Provides supportive counseling, crisis intervention, and referrals to appropriate community resources. Assists with hospital-sponsored support groups.
6. Collaborates with all members of the healthcare team to develop, manage and communicate discharge plans. Demonstrates awareness of the impact of managed care population and practices.
7. Functions in a liaison role between the hospital and community in making community resources available to the patient and family.
8. Participates in programmatic development and implementation. May provide assistance with supervision of students for graduate/undergraduate field instruction. Assists with orientation and training of new social workers or case managers.
9. Participates in interdisciplinary teams to improve patient care. Provides professional education to staff and community.

Follows safety standards in all aspects of performance of the above functions, reports/removes unsafe equipment and attends safety education sessions.

Qualifications :

This position requires a Master's of Social Work degree with 3 years experience in the acute care hospital setting or three years post-Master's experience in related medical settings. Requires Emergency Dept experience. Knowledge of community resources, including AHCCCS and long term care is essential. Must have excellent communication and human relations skills and be able to establish and maintain effective working relationships with other healthcare professionals. State certification and Academy of Social Workers certification preferred. Must be able to work flexible schedule and take call.
