Social Worker, MSW II - CHNP/Schools Program

Department Psychiatry Job Posting Description Provides clinical social work services to assist patients and families with personal, family, medical and environmental difficulties. Performs psychosocial and risk assessments, assigns diagnoses and develops and implements interventions and treatment plans. Participates in planning and implementation of discharge and outpatient service plans and collaborates with health care team and community agencies to plan, implement and follow up on treatment and provision of needed services. MSW and Massachusetts licensure required.
