The Social Worker works to strengthen children and families ability to cope, while building on the family's strengths to preserve their bonds, so all family members can live with vision and hope.
JOB DESCRIPTION
(Job Title: Social WorkerLBSW/LMSW/LMFT/LPC)
Credentials: License to practice in the State of Kansas. Bachelors degree in Social Work or a Master's degree in Social Work, Psychology, Marriage and Family Therapy or Counseling from an accredited college or university. Two years experience working with children and family. Must be 21 years of age. Demonstrate effective written and verbal communication skills. Understanding of family centered practice, genograms, and ecomaps. Understanding of child and adolescent development. Must be a team player. Demonstrates competence and continues training in age-specific competencies for infant through adult clients.

Description: Implements SFA policies and follows directives as required. Follows and adheres to all pertinent SFA Standard Operating Procedures (SOP's), rules, personnel policies and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, regulations, and contractual terms.
Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
Ensures clients rights are protected.
Makes sure all clients have received their rights and maintains a signed copy in each file.
Is knowledgeable of and follows all safety procedures.
Will participate in a minimum of 40 hours in-service education per year.
Completes initial and ongoing assessment of children and their families. Gathers information from the child, family, and collateral's to complete the social history and family assessments.
Responsible for initial placement of children within 4 hours of referral, and subsequent placement. Researches possible relative placements, giving them priority, whenever viable.
Participates in family meetings.
Develops and implements case plan and permanency goals with children and families, ensuring that goals are reached within established time frames.
Coordinates and arranges needed services for the child and family, including visitation/interaction.
Assists families to establish a support network with the support of family, kin, and community.
Takes on-call rotation.
Provides counseling to children and families, preparing them for moves, reunification or other appropriate permanency goals.
Provides intervention for clients in crisis situations.
Liaison with SRS,courts, schools, other providers, and communities.
Reports critical incidents to SRS verbally within 12 hours, written within 24 hours.
Completes reports for court and participates in court proceedings.
Completes and maintains written documentation in child/family's case record accurately and within established time frames.
Supports resource families to maintain placements and help children work toward permanency.
Make referrals to adoption contractor when appropriate, completing necessary documentation and working closely with adoption worker to identify/select an adoptive family.
Participates in supervisory conferences and team meetings, sharing pertinent information about children/families progress, collateral contacts and relationships with other agencies/communities.
