Relief, day shift, various days, occasional weekends

Description:

HOME HEALTH PHYSICAL THERAPIST is a Licensed Physical Therapist providing therapy to Home Health patients in accordance with a written care plan and written physician's order.

1. Maintains an in-depth knowledge of principles, practices, standards and techniques and demonstrates knowledge in accordance with SCMC policies and procedures within pertinent laws and regulations in the following areas:
a. Physical Therapy Licensing Board in the State of Oregon.
b. Physical Therapy evaluation and treatment to achieve desired outcomes.
c. Staff consultation and training.
d. Community resource knowledge and referral process.
2. Care Team: Functions as a member of the Interdisciplinary Care Team to assure home care is accomplished effectively and efficiently in a cost-effective manner. Participates in clinical care conferencing.
3. Documentation: Gathers data and accurately documents information in a timely manner demonstrating compliance with home health regulations.
4. Patient Education: Participates with team members in facilitating patient's and family's learning throughout the course of home care. Reinforces patient's continued health care through teaching and/or referral to community agency follow-up.

Requirements:
Education: Bachelor of Science or Masters degree in Physical Therapy from a collage or university accredited by the American Physical Therapy Association.

Licensure: Currently licensed in the state of Oregon. Oregon drivers license

Experience: One-year experience as hospital or rehabilitation therapist required. Home Health experience helpful.

Computer Literacy: Ability to learn and apply basic computer skills including locating and opening documents, completing forms, opening and sending e-mail messages.
